Form聽W-2, Wage and Tax Statement

驰辞耻谤听2022聽Form聽W-2聽is now available聽if you elected to receive the form electronically. To access your聽2022聽Electronic Form聽W-2:

• Login to聽
• Go to Personalized Links, click 鈥淲ork @ 麻豆原创鈥�
• Click 鈥�奥-2听厂迟补迟别尘别苍迟蝉,鈥� log into Eagle Service, and select year 2022.

Your Form聽W-2聽will load in your web browser as a PDF file. If the file does not display correctly, try using a different browser such as Apple Safari or Google Chrome.

If you opted not to receive your Form聽W-2聽electronically or did not complete the consent form聽before January 17, a paper version will be mailed to your home address and postmarked by January 24.

If you would like to change the method by which you receive your Form聽W-2聽in聽future聽years:

• Login to聽
• Go to聽Personalized Links, click 鈥淲ork聽@ 麻豆原创鈥�
• Click on 鈥�奥-2听厂迟补迟别尘别苍迟蝉鈥� and log into Eagle Service
• Click on 鈥淩eceive my W-2 only in electronic format鈥� and hit 鈥淪ave.鈥�

Form聽1095-C, Employer-Provided Health Insurance Offer and Coverage

In addition to the聽W-2, faculty and staff who were eligible for 麻豆原创-sponsored medical coverage at any time during calendar year 2022 will receive a聽Form 1095-C聽mailed to their home by the end of February. The form is required by the Affordable Care Act (ACA) and provides documentation to support your income tax filing for 2022. Kaiser medical plan participants also will receive a Form 1095-B directly from Kaiser.
