FORM W-2, Wage and Tax Statement听

驰辞耻谤听2021听Form W-2听is available now听if you elected to receive the form electronically. To access your听2021听Electronic Form W-2:听

• Log in to听听
• Go to Personalized Links, click Work@麻豆原创听
• Click W-2 Statements and select year听2021听

Your Form W-2 will load in your web browser as a PDF file. If the file does not display correctly, try using a different browser such as Internet Explorer, Apple Safari, or Google Chrome.听

If you opted not to receive your Form W-2 electronically or did not complete the consent form听before January 20, a paper version will be mailed to your home address and post marked by January 31.听

If you would like to change the method by which you receive your Form W-2 in听future听years:听

• Log in to听
• Go to听Personalized Links, click Work@麻豆原创听
• Click on W-2 Statements听听
• Select either听Opt-In听(electronic) or听Opt-Out听(paper) and click Submit听

FORM 1095-C, Employer-Provided Health Insurance Offer and Coverage听

In addition to the W-2, faculty and staff who were eligible for 麻豆原创-sponsored medical coverage at any time during calendar year 2021 will receive a听Form 1095-C听mailed to their home by the end of February. The form is required by the Affordable Care Act (ACA) and provides documentation to support your income tax filing for 2021. Kaiser medical plan participants also will receive a Form 1095-B directly from Kaiser.听
